Important Information

  • If you are a new patient or are being seen for your annual exam, please fill out the Patient Paperwork found below and email it to us at at least 24 hours prior to your visit. Please arrive 15 minutes early to your appointment. 


  • If you are unable to complete the paperwork ahead of time, please arrive 30 minutes prior to your scheduled appointment time to fill out the clinical paperwork in office.


  • If you are being seen for a telemedicine visit, please ensure that you are not driving or in your car. The video works best when connected to WIFI. If you are driving, we will need to reschedule your appointment for your safety. Please note, we screen share to show your results, especially for the Genetic Protocol test and it is very difficult to see on a phone. Whenever possible, please use an iPad or a laptop for telemedicine visits.


  • You will need to have your insurance card, driver's license, and credit card present at the time of your visit to put on file. 


  • Payment will be due at the time of your appointment. Please note that this amount will be determined by your insurance or our cash pay prices. 

  • Please bring all previous records that pertain to your visit with you to your appointment or have your physician fax these records directly to our office fax at (480) 970-1938. ​Please call in advance to verify that we have received your records so that we may avoid a possible delay in your appointment time. If your previous physician requires written consent, you can download the Records Release form above. 

  • If you would like to be placed on our waitlist, we recommend that you complete and return our Patient Paperwork as soon as you’re able. We will be unable to call you from the waitlist until we have received your paperwork.
